To request an appointment, please fill out this form completely. We will be in touch as soon as possible. We will reach out via phone; if we cannot reach you, we will only leave a voicemail. Please review the New Patient Information selection below.

PLEASE NOTE!

  1. We can only accept clients in the following states: Texas, Iowa, New Mexico, Colorado, Kansas, and Missouri. 

  2. We do not prescribe or continue controlled substances. 

  3. We are only available through telehealth via live chat, telephone, or video chat. 

  4. The current wait time for an appointment is about three days.

  5. We do not schedule appointments until our office receives insurance verification.

  6. We are very strict on our attendance policies out of respect for our clients. You are required to give 24 hours notice for any cancellations or rescheduled requests. If you reschedule, we can't guarantee an appointment will be available the same day or week. We only give a 10-minute grace period on all new client visits. We will never "fit you in" or run into another client's appointment time; you must reschedule. A fee will apply.

  7. We do not tolerate disrespectful behavior from clients, providers, or family. Any behavior deemed rude will be grounds for discharge from our care.

  8. Treatment compliance is an integral part of your treatment. We will work with you to determine the best treatment plan for you. Still, repeated failure to follow our treatment recommendations (including not getting labs and not picking up prescriptions) will be evaluated on a case-by-case basis. It may result in discharge from our care.

New Patient Information

Clinical Communication 

All clinical communication will be done through Spruce, an app you can download here: https://spruce.care/scottpsychiatricconsultantsllc. When you become a patient, you will get a notification to join our practice on Spruce. Spruce ensures that all messaging between you and your provider is HIPPA-compliant and secure. We do not utilize any client portal for messaging. Initially, we may only contact you through text or e-mail to establish you as a patient. After that, all correspondence will be required to go through Spruce.

New Patient Intake Process

Step One - Consultation

When you request an appointment by phone, e-mail, through our website, or Alma, we will contact you to gather some basic information. So that you are prepared, we will ask for the following details: 

  1. Full Name

  2. Date of Birth

  3. Phone

  4. E-Mail Address

  5. Full Address

  6. Controlled Substance History

  7. Insurance Company, Member/Group ID, Primary Subscriber, Primary Subscriber Date of Birth/Address

  8. Presenting Problem

Step Two - Paperwork

After receiving a verification of benefits from your insurance company, a chart will be created and new patient intake paperwork will be e-mailed to you. All new patient paperwork is done through the client portal. Once the consultation is completed, you will receive two e-mails: A request to sign up for the client portal and a request for you to fill out the paperwork. If you plan to use insurance for payment, you may receive a request from Alma to sign up for AutoPay. (Alma is how we bill insurance for services provided if you have Cigna, Aetna, or UnitedHealthcare/Optum). You will also receive a request to download Spruce (app) and join our practice. See Clinical Communication Message above. If you have any questions, please get in touch with our office at (972) 945-8840. Our client portal is specific, so make sure that the password you choose meets all the requirements; if you have trouble, please get in touch with our office.

Step Three - Scheduling First Visit 

We will contact you to schedule a comprehensive psychiatric evaluation after ALL paperwork is received and processed. We will discuss presenting symptoms, background information, and treatment options. If labs are needed, these will be ordered during this evaluation. No prescriptions will be issued until all necessary labs come back and the provider has reviewed them. You will get a message from us through Spruce (our clinical communication app).

Step Four - Follow-Ups

Congrats, you are now an established patient with SPC. After the initial psychiatric evaluation and after medications are prescribed, a two-week follow-up will be scheduled. If things are going great, and there are no concerns from either patient or provider, follow-ups will switch to monthly. Follow-ups will be needed at least monthly (no exceptions) to reassess your progress and to evaluate for any changes in your condition. Follow-ups are required; no prescription refills will be sent without a follow-up appointment. 

Important Notices!

1. Self-Pay Rates: $250 for initial psychiatric evaluations and $125 for follow-up visits. All fees are due before service; we will not allow "carried balances."

2. You must be on time for all appointments to respect other patients. All visits are telehealth via secure video chat, secure messaging, or secure phone calls. We only give a 5-minute grace period for established client appointments and a 10-minute grace period for new client appointments. You must provide 24 hours' notice for any rescheduled appointments. Late cancellations and no-shows will be charged according to our policy.
